My current assignment has me working for Sacramento County’s Primary Care Center as a Patient Care Coordinator/Manager. I am responsible for reaching out to patient’s that are considered to be high risk/high utilization. I have been working with the insurance companies to coordinate our mutual patients care, including requesting case managers for those patients that are experiencing homelessness, mental health issues and language barriers. I often assist with coverage for the positions of Triage RN and Phone Triage RN.

I worked as a Lead Clinical/UR nurse. I was responsible for performing a nursing assessment on patients upon admission and as needed for Partial Hospitalization/ Intensive Outpatient Programs. I oversaw two per diem nurses. I noted all orders written by the Psychiatrists. I was responsible for ordering lab work as needed. I was the team lead for any patients that were stepped up to a higher level of care. I was also responsible for coordinating authorization from the patient’s insurance company for their stay. I kept track of patient’s daily attendance for multiple programs. I worked closely with the Psychiatrists, Therapists, and Insurance Case Managers to manage the patient’s care.
