Thank you for taking an interest in applying with the Revelation Of Faith T; raining Institute. In order to process

your application, the following items must be received by our office:

Two current photographs (passport size)

Registration fee $75.00

* One Minister's Recommendation Form (not required. for those applying for leadership classes only)

* Ohne Relative's Recommendation Form

* One Recommendation Form

The following information is also required:

Itinerant Ministers:

* Itinerary (current schedule of meetings for the previous three months and upcoming three months)

Children, Youth, Music, Associate Pastors:

: Letter from your pastor or head of the ministry for which you work detailing your ministerial duties and

=} responsibilities (please include pulpit responsibilities)

Pastors: A \etter signed by 3-5 members of your congregation attesting to your position, church start date,

location, service times, and schedule.

All Other Applicants Applying For Just Leadership Class:

¢ Letter outlining your Moral and/or Christian activities and responsibilities.

After your application has been received your HDIC Director will contact you and conduct a telephone

interview.
