ViaQuest Hospice: 6/2024 to Present – working 20 to 40 hours per week as PRN

Hospice Home Care Social Worker provides support and assistance to individuals and families facing terminal illness or end-of-life care. Assists patients, family members, and healthcare providers to ensure that patients receive the highest quality of care and that their physical, emotional, and spiritual needs are met. Meet with patients and their families and help navigate healthcare system, providing guidance and support throughout the end-of-life journey. Offer counseling, bereavement support, and assistance with financial and practical issues. Work in variety of settings such as hospitals, hospice facilities, and patients' homes. I provide emotional support and practical assistance to patients, family members, and caregivers, as well as to work collaboratively with other members of the healthcare team. Assist patients and families understand the end-of-life process, provide information about available resources, and support patients and families as they make difficult decisions about care.

Springfield Regional Medical Center: 4/10/2020 to Present

Case Manager: Initiates discharge planning on admission through patient/family interviews and assessments. Coordinates the plan of care with the primary care nurses, physicians, and other members of the healthcare team. Gathers information about patient’s home environment, family, relationships, and social-economic conditions to assist, improve or restore patients' capacity for social functions. Makes appropriate referrals for all abuse, neglect, homeless or nursing home placement patients. Serves as patient/family advocate by evaluating and arranging necessary funding, treatment alternatives, timeliness, and coordination of health services. Contacts appropriate third party payers to obtain appropriate certification/re-certification of hospital services.

Collaborates with interdisciplinary team, community agencies, physicians, patients, and families to develop appropriate discharge plan. Coordinate communication of plan and assists with monitoring interdisciplinary documentation in the medical record.

Equitas Health: 3/2018 - 4/2020

Medical Case Manager: Provide high quality case management for clients and their families by assisting them to access medical services, health insurance, Ryan White benefits and other resources and services to improve health outcomes, housing stability. Conduct comprehensive psychosocial assessments for individuals with HIV seeking services at intake and complete update assessments semi-annually and as needed. Develop and monitor and evaluate individual case plans for each assigned client at intake, bi-annually, and as needed. Function as a central and primary access point for financial assistance programs, including but not limited to Ryan White, HOPWA and other assistance programs as appropriate.
