Objective

I’m looking to relocate to the San Diego area and gain employment as a Family Nurse Practioner in a setting where I can utilize my advanced nurse practitioner education, clinical skills and 22 years as a registered nurse to provide high quality, patient-centered healthcare. Responsible for creating and maintaining complex care plans for ER patients; worked with local PCP’s and specialty care providers to decrease frequency of ER use. RN Patient Flow Coordinator: Concord Hospital, Concord, NH July 2009-January 2012

Coordinated the bed management process for the busiest ER in the state, ensured timely throughput for the admission, transfer, and discharge process. Facilitated direct admissions from the community and transfers from other hospitals; effectively communicated with interdisciplinary team members to balance patient care needs with staffing availability to ensure appropriate patient placement.
