I was part of the St. Mary clinical team (RPh/RN/MD) that was awarded the Innovative and Collaborative Practice Award by the Pennsylvania Society of Hospital Pharmacists (PSHP) for the new pharmacy protocol governing the prescribing of Coumadin® (warfarin sodium) after joint replacement surgery. Coumadin is prescribed after hip or knee replacement surgery to prevent DVT. The pharmacy managed Coumadin protocol was developed collaboratively by the St. Mary Pharmacy and Orthopedics department.
As an RPh under this protocol, I managed the dosing of Coumadin and monitored patients not only during the post surgical recovery period, but also after discharge from the hospital. During the in-home recovery process, I tracked the patient’s laboratory results and independently adjusted doses of Coumadin and contacted patients to discuss adjustments in therapy.
