It was a great experience with this group . I had a group of patients – walk in, appointment patients, coming for treatment, counseling and much more. Although the mode of work was same as before but during this tenure I got more skilled in diagnosing and treating a variety of human diseases and injuries. It added to the ability to provide psychological diagnosis, treatment, and rehabilitation for patients with mental and physical dysfunction .

My job in the clinic was very challenging. I had to handle all the emergencies of my own. Most challenging were pediatric cases and of course adult emergencies. Pediatric cases like trauma, acute abdomen, febrile seizures, rashes, foreign body ingestion etc were mostly seen individually. Even identifying adult emergencies was not so easy. My job in the hospital was to see all walk in patients. Most challenging among these were the ones who presented with acute cardiac emergencies. Since I had previously worked in a cardiac set up I had basic idea of cardiac emergencies. I treated patients with acute coronary syndromes like unstable angina, non-STEMI, STEMI. In addition to the cardiac I handled patients with non-cardiac emergencies like acute severe asthma, acute exacerbations of COPD, PE, acute abdomen, upper GI bleed, hypoglycemia, DKA, electrolyte imbalance, status epilepticus and trauma.

Since Dubai is an emerging international city and thus becoming venue for various medical conferences which gives me an opportunity to be a part of these events.

Here also I was assigned to manage patients with conditions as acute MI, chronic stable angina, congestive cardiac failure, hypertension, acute exacerbation of COPD, bronchial asthma and other emergencies. During the above tenure I got to know about rural people of the state. In addition to the acute cases I were to screen patients for cardiac disease mainly Rheumatic heart disease. This was the time when I gained experience in working up patients with Rheumatic heart disease, Congenital cyanotic as well as a cyanotic heart disease

During this tenure i worked in child and maternity section. It was an OPD base schedule. Besides seeing the routine cases in obstetrics i dealt with a neonates and care was mainly taken about the hygiene of mother and baby during feeding.

During my tenure as resident Cardiology I ran an independent daily clinic screening patients on a walk- in basis. This involved detailed history taking viewing X-rays, interpreting ECGs, investigations etc. referral to senior cardiology colleagues and other Consultants. I also underwent an intense academic programme in cardiology viz arrhythmia recognition and management, conducted Treadmill testing and interpretation, holter monitoring and reviewing, competently interpreting Chest X- Rays and laboratory investigations. On call commitment consisted of 1:4 Rota for cardiac cath. and care of CCU patients. I became proficient in the use of cardiac defibrillators, infusion devices and volumetric pumps. In addition I was responsible for postoperative follow up of cardiac surgical patients and pacemaker follow-ups.

My on call Rota based on a 1:4 schedule required emergency management of Cardiac and general medical patients.

The Institution regularly conducted Cardiac Camps in very remote regions of my state and I duly attended eight of these camps. I also responsibly discharged my administrative duties in so far as streamlining rotas for doctors and officiated for the hospital administrator as and when required. JUNIOR RESIDENT

During this period I worked on the wards as assistant to registrars and consultant gynecologists, admitting patients for elective LSCS and also surgical emergencies (obstetrics and Gynecology) .In addition to this I had a theatre duty for organising lists and post operative care in the recovery wards apart from assisting in major procedures.

The rotation is through 6 hospitals with total bed strength of 2300. This involved clerking and admitting and workup of patients under supervision of senior colleagues and Consultants.
