Announcement Number: AD -16-NMB-1632350-BU-SR

I have experience with Neurosurgery and working with Deep Brain Stimulators for both Parkinson's Patients and Essential Tremors Patients. It has been amazing to watch how such a change in controlling their excessive movements can make such a difference and improvement in their quality of life.

Professional Objective

To practice in the capacity of a Family Nurse Practitioner by providing the best quality healthcare to my patients and their family members. My strengths are patient advocacy, patient education and assessment. My 32 years of ICU, ER, Community Outreach and House Supervision has prepared me for any future challenge. I am up for any challenge and thrive in a position in an environment whereby I can learn, grow and at its best make a difference in the lives of those I touch.

After my deployment with the Red Cross, I was involved in a motor vehicle accident while driving in the fog was involved in a serious motor vehicle accident which involved multiple injuries including injuries to my resulting in dental extractions and repair. After multiple surgeries and am healthy and able to return to work without restrictions. During the recovery of this accident, I had home care for 3 months. Although this experience was traumatic, it did give me a chance to experience firsthand how healthcare can be brought into a patient's home and interact with family members in a

necessary and positive way. In addition to being a member of the Washtenaw County American Red Cross National Disaster Team, I am now a member of the National American Academy of NP Disaster Task Force.

I have enjoyed my career as an RN and Nurse Practitioner and know that through patient advocacy, education and experience that I can make a difference in the lives of my patients and their families. I have a solid work ethic and am enthusiastic and motivated by the opportunity I have to teach patients about self care and ways to adapt healthy life styles. I know I would be an asset to any team.

Solvis Medical Staffing -13 week locum tenum position

PLLC- Auto Accident Rehabilitation

December 2011 to March 1, 2012 13 week contract

In this position I worked as an FNP seeing patients who have been injured in automobile accidents. Their complaints are predominately in both spinal cord, head and extremities injuries. I found out after the assignment having not seen the anesthesia physician that hired me in 13 weeks that he needed an NP as he had lost his DEA

license for some type of violation. There were many other infractions that I felt fell below the bar on safety, quality and ethics so after completing my 13 weeks I resigned.

Washtenaw Community College of Nursing---responsible for clinical education of nursing students on ICU step-down unit at St. Joseph’s Hospital (Ann Arbor, Michigan 2006) I left Washtenaw even though I loved teaching the students because I needed a full time job and they only had part time faculty positions

Arizona Center for Neurosurgery, Phoenix, Arizona---Responsible for care of neurosurgical patients in ICU as a Neuro-surgery nurse practitioner in addition to RN first assist in surgery for brain tumors, Parkinson’s Disease, MS, essential tremors, and trauma. Was in surgery Mon., Wed., and Friday and in clinic Tues and Thursday. Was responsible for regulating deep brain stimulators post insertion for Parkinson's disease

and Essential Tremors (2005). I made rounds on approximately 25 level 1 ICU patients per day. Taking care of those patients and seeing their victories over their disease processes was one of the highlights of my careers. Those patients that had brain tumors and could not be helped also touched me in a way that I will never forget.

Even though many patients were not successful in beating their disease process, I know in my heart that their families will always remember me for the difference I made in helping them understand their loved one's death, learn how to celebrate the little victories and remember the outstanding experiences that they had over the years. To be able to laugh with a family when they feel a little sense of joy and then to cry with them upon defeat changes one in a way that nothing else can compare. To that end, I am ever indebted to the experiences that they have given me in return, as it has made me the caring, compassionate, can-do, enthusiastic, passionate health care professional that I am today.

I realize my resume is very long compared to most but I felt the need for you to have a complete understanding of both my professional career as well as my period of absence. I also want you to know that while I was not working I read 6 journals per month, continued my memberships in all my professional organizations, attended a

couple of local conferences, renewed both my CCRN and CEN which all require 100 CEUS each. I renewed my PALS, ACLS, BLS all at my own cost and most importantly renewed through CEU and conferences my FNP-bc.,. All of these were very expense and it was hard to come up with the financial resources as I was not working but they are too important to me to let expire. I do not have children and nursing and taking care of patients gives me a satisfaction that money can't buy. There are nurses that I have worked with in the past that haven't taken a single class other than pals or acls in 40 years. That is far below the bar of what the patients deserve.

Thank you for your patience and consideration for this position.

Respectfully Submitted,

Sandra M. Jones-Yapp RN, FNP, bc., MSN, MBA, CCRN, CEN, ACLS, PALS, BLS
