•Performs field and office based intensive social services with individuals aged 15-30, whom have been diagnosed with their first episode of psychosis within the last 2 years.
•Works as part of an intensive team with a small caseload (20-30) with the goal of stabilizing these individuals in the community and helping them and their families better understand psychosis.
•Responsibilities include engagement (including Motivational Interviewing skills to engage the individual), providing routine case management, skills training & development, psycho-social rehabilitation, patient & family education, referral to appropriate state and federal benefits, medication education, supported housing, and supported employment.
•Coordinate with the behavioral health clinics, as part of discharge or step-down of service packages, for psychiatric services, counseling, and substance abuse services, as needed.
•Complete detailed, yet concise progress notes on all services provided during or immediately following the session.
•Work closely with various community collaborators and referral sources, including but not limited to municipal police forces, court and jail liaisons, and other LifePath Systems personnel.
•Increase awareness for clients and their families of issues relating to psychosis, to ensure clients receive the services they need to remain stable and functioning in the community.
•Complete documentation in the EHR.
