My Duties:

I coordinated Mental Health Act assessments within the legal parameters of the Mental Health Act (UK), which requires two medical recommendations and an Approved Mental Health Professional to section patients under the Act in the community settings, hospital or police stations. If necessary, I obtained warrants from the Magistrate’s court or out of 4

hours from a Magistrate’s home, execute the warrant with police, ambulance and doctor to convey a service user to a place of safety or into a psychiatric hospital. Then jointly with other professionals plan for the patient’s treatment and care. I completed the community treatment order (CTO) following an agreement on conditions attached to the CTO by the Responsible Clinician (RC) about the service users under the Mental Health Act 1983, (amendment 2007).

I wrote the mental health review tribunal reports, submitted these and attended hearings on behalf of the social services.

I conducted risk assessment, social circumstance assessments, prepared care plans on the basis of identified care needs of the service users. The assessments considered service users’ capacity, abilities, care needs, including entitlement, accommodation needs, support availability and accessibility, whether or not the service user’s care needs could be met in the community setting or required an admission to hospital or residential care home. I have supervised/mentored students and AMHP candidates. I also performed rota duty as a member of the Community Mental Health Team, acting as first point of contact.

I have completed social need assessment and reviews, FAIR ACCESS TO CARE SERVICES

(FACS), Eligibility Band to provide the help and support that the service users require. I applied for home care support, direct payment. I applied and arranged for residential placements and reviewed these.

I have completed carer’s assessment and provide carer’s support, including helping to obtain carer’s grant, arranging a break and counselling. I helped service users apply for community care grants. If they receive this, I may support them in buying the products they may want.

I have dealt with the adult protection; safeguarding issues for the vulnerable services users, I asses/investigate the service user’s circumstances, risk factors, trying to protect service users from abuse, (such as sexual, physical, mental, emotional, or financial abuse) neglect and discrimination from others, with help and support from appropriate services or authorities. I was care co-ordinator for patients under the Care Programme Approach for over ten years before moving into the AMHP team. I had a caseload of 30 to 35 service users to carry on with clinical work, including carrying out individual interventions/assessment and provided care/support depending on their needs as daily basis. I have worked in a multi-cultural and multi-disciplinary team and attend/participated MDT meeting twice a week.
