•Internal Medicine service, Hospital General de Macas, Ministry of Public Health, Ecuador helping the indigenous ‘Shuar’ communities in Ecuador under less than ideal conditions to practice medicine with limited resources, on a personal humanitarian mission. It was a gratifying experience helping these communities living in the deep pockets of the Amazons with no terrestrial access and no regular conventional electricity supply. Among the services that we provided, we regularly carried vaccines, insulin, anti-venom for snakebite victims with improvised cold chain conditions of transport and other supplies. We educated them in the need for regular and continued follow up and therapy for the frequent chronic conditions among them like DM and chronic liver disease. We added vaccination programs for the elderly with pneumococcal vaccines. We organized health care for patients with HIV-AIDS. We organized airlifting patients with acute conditions to the better-equipped provincial and regional health care centers. We taught basic principles of first aid and emergency care to the local health care workers.
