SUMMARY OF QUALIFICATIONS
My education and experiences have allowed me to develop skills in providing inpatient and outpatient Advanced Practice Nursing care to persons diagnosed with mental illness as well as persons diagnosed with Co-Occurring Disorders such as Substance Abuse and Developmental Disability. As a Family Psychiatric Mental Health Nurse Practitioner, I have provided services that included Psychiatric Evaluation, Physical Assessment, Treatment Planning, Medication Initiation and Monitoring and referral as indicated. As an administrator I have been responsible for new program development and development of continuous quality improvement programs which included maintaining national accreditation and state licensure of organization programs. I have provided consultation services to state organizations and associations to assist them with preparing for both the Rehabilitation Accreditation Commission and Joint Commission on Accreditation of Healthcare Organizations accreditation process. My consultation services also included assisting organizations develop and implement the State of Illinois Medicaid Rehabilitation Option program.

Conduct Initial Psychiatric Evaluations and develop plan of care, including, but not limited to referral to Inpatient Psychiatric and/or Chemical Dependency Treatment, initiation/adjustment of psychiatric medications, and brief Individual Therapy, within the Pediatric to Geriatric population in the Outpatient setting. Conduct Initial Psychiatric Evaluations and/or Physical Assessments and provide medication monitoring within the Pediatric to Geriatric population in an Inpatient setting. Provide 24-hour On-Call coverage one week each month for the Inpatient setting.
