Case manager of 10-20 chronically homeless/mentally ill, many with co-occurring substance abuse diagnoses, individuals in the Washington area that are in need of housing and the extensive ACT services offered by this agency. Duties include: Full case management of the consumers connected to the act team by providing flexible, community based services that are designed to promote the stability, recovery and community reintegration of these formerly homeless individuals, which may include finding and assisting the consumer to move into an apartment, assisting with daily living skills, the making of and accompanying consumers to appointments for support, connecting consumers to available services offered by the DC and US government. All around assistance with any issue need that may occur in ones life. In addition to the case management duties I also provided assessment, intake, individual and group counseling, liaison between client and court system, treatment planning, documentation and educational support as it relates to Substance Abuse issues.

Assistant Coordinator- September 2005-January 2006

Assistant Coordinator of a men’s transitional house for addicts who have completed substance abuse treatment at SOME inpatient center. Duties included: Supervision of night and weekend monitors as well as the program cook. Do individual and group counseling and perform complete case management services to residents. Be a part of the program treatment team that make admission decisions as well as develop individual treatment plans for program residents. Enforce program requirements, house rules and monitor the program facility to ensure safe and efficient operations. Maintain complete client records as well as overseeing the client bank account and finances. Oversee the entire program in program coordinator’s absence.

SAFE HAVEN OUTREACH MINISTRIES

Treatment Specialist/Primary Counselor - June 2004 to September 2005

Primary counselor for 12-20 co-ed clients in an inpatient/residential treatment/transitional setting that are primarily dual diagnoses and referred by the criminal justice system. Duties include individual counseling, didactic and therapeutic group counseling, assessment, treatment planning, clinical staffing of residents to determine their needs, monitoring day to day activities as well as making referrals.

THE NEXT STEP PROGRAM/FOUNDATION FOR A CONTEMPORARY MENTAL HEALTH

Case Manager/Contract Coordinator-December 1999 – June 2004

Case manager and contract coordinator of 70-80 male and female clients in an outpatient setting referred by the federal criminal justice system. Duties include intake, assessment, orientation, contract liaison between client and criminal justice system, mentoring of masters interns from local universities, individual counseling, didactic and therapeutic group counseling, treatment planning, training of other staff members, monitoring and reporting of urinalysis results, and referrals.

Supervision of case managers, reviewing of charts, shift coordinator and mediator in disagreements. Full service case management of approximately 7-10 alcohol and drug dependent men and women living in a residential setting. Duties included intake, assessment, orientation, individual and group counseling, family contact, treatment planning, education and referrals.

Addictions Counselor III- July 1996-October 1997

Primary counselor of approximately 40-48 adult men and women who are opiate dependent and receiving daily medication of Methadone in an outpatient setting. Duties included assessment, intake, individual and group counseling, liaison between client and court system, treatment planning, education and referral.
