Dear Abby Anderson

This letter is to inform you of your current employment status with Lighthouse Autism Center after missing two consecutive scheduled shifts without prior communication. Per Lighthouse Autism Center’s Attendance Policy, “When a team member does not show up for their job and does not communicate with their employer. Two consecutive NCNS would result in job abandonment.” Due to your absence on 8/21/26 and 8/24/2026, this constitutes job abandonment in accordance with company attendance policies. (P. 22-25) We are accepting the voluntary resignation of Abby Anderson effective today, 8/25/2026 Due to this repeated policy violation, your employment with Lighthouse Autism Center is terminated effective August 25, 2026.

If you elected medical, dental, or vision insurance, you would remain with coverage through 8/31/2026. At that time, you will be offered to elect to continue the company’s medical, dental, or vision insurance at an additional cost. You also may be eligible to port or convert your basic and optional life insurance coverage(s). To be eligible for port coverage, you must have been actively at work on the date your employment ended. You must complete an application and apply for these options within 31 days of your coverage termination. To obtain an application, please contact Prudential at 1-800-***-****.
