• Provide person-centered care and assistance with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, feeding, and comfort care while maintaining residents’ dignity, privacy, and independence.

• Support residents living with dementia through therapeutic communication, patience, reassurance, and gentle redirection strategies to reduce distress and promote emotional well-being.

• Assist residents with safe mobility, transfers, and repositioning using proper body mechanics and mechanical equipment, including Hoyer lifts, sit-to-stand lifts, bath tub lifts, transfer belts, wheelchairs, and walkers, in accordance with care plans and safety protocols.

• Observe, document, and report changes in residents’ physical, cognitive, emotional, and behavioural status to nursing staff to support timely interventions and individualized care.

• Provide palliative and comfort-focused care by supporting residents’ personal needs, emotional well- being, comfort, and dignity during advanced stages of illness.

• Perform skin observations during personal care routines and promptly report concerns to support skin integrity and pressure injury prevention.
