Manage and own a 6 operatory General Dental practice with an emphasis on fixed prosthetics. The office is only in network with Delta Dental Premier Provider. It is a Fee for service practice. Providing all aspects of dentistry including cosmetic dentistry, implants, laser dentistry, crown and bridge, periodontal treatment, extractions, dentures, digital radiography, dental photography, emergency care and treatment planning. 2 hygienists on staff. Ownership duties included but certainly were not limited to; training of staff, production/collection evaluation, payroll, accounts payable, operations, dentistry, and hiring of staff. Maintain a 3-4 day per week patient schedule. This practice was purchased from a retiring dentist.

10/2012-Present: Owner/General Dentist Muncie Dental Care & Denture Center Muncie, IN

Manage and own a 4 operatory General Dental practice with an emphasis on removable prosthetics. Providing all aspects of dentistry including cosmetic dentistry, implants, laser dentistry, crown and bridge, periodontal treatment, extractions, digital radiography, dental photography, emergency care and treatment planning. Prosthetic Denturist and fabrication of our own dentures in our lab. Ownership duties included but certainly were not limited to; training of staff, production/collection evaluation, payroll, insurance contract negotiations, accounts payable, operations, dentistry, and hiring of staff. Maintain a 3 day per week patient schedule. This practice was purchased from a widow of a dentist, and the practice was not maintained well. This was basically a start-up practice from scratch.

High volume of patients covered by both insurance and Medicaid. Treatment plan and treat all patients and walk in patients. Provided all aspects of dentistry including root canal, crown and bridge, Invisalign, extractions and restorative dentistry. High producing / volume office.

09/2010-08/2011: Managing Clinical Director/ Dentist Aspen Dental Management Indianapolis, IN

Managed and operated a new location within the Indianapolis area. Diagnosed and treatment plan patients and perform all aspects of general dentistry with an emphasis on extractions, dentures, and crown and bridge. Consistently in the top 10% of dentists nationwide in the company with regards to treatment plan values and acceptance of treatment plans. I was able to work efficiently and balance a large patient load. I also provided training to underperforming dentists working at the company and became a training facility to dentists who were new hires.

Managed and owned a 14 operatory General Dental/Multi-Specialty dental practice consisting of 8 Doctors and 18 staff members. Specialty Services include Orthodontics, Endodontics, Periodontics,

and Oral Surgery. Providing all aspects of dentistry including cosmetic dentistry, implants, crown

and bridge, periodontal treatment, extractions, digital radiography, computer simulation and demonstration, dental photography, emergency care and treatment planning. Annual production reached $2.9 Million. Ownership duties included but certainly were not limited to; dentist training, production/collection evaluation, payroll, insurance contract negotiations, accounts payable, and

hiring of staff. Maintained a 3-4 day per week patient schedule. Sold practice August 2010.

02/2000-04/2006: Founder/Owner/ Managing Dentist Plaza Family Dental Group Carlsbad, CA

Managed and operated a 12 operatory General Dental/ Multi-Specialty dental practice consisting of 7 Doctors and 12 staff members. Specialty services included Endodontics, Periodontics, and Oral Surgery. This Practice was mostly absentee ownership. Annual production reached $1.9 Million. Responsibilities included a very limited patient schedule of 2-4 days per month. Ownership

duties included production and collection evaluation, doctor and staff goals, dentist training, insurance negotiations, accounting and business duties. Sold practice in April of 2006 to my associate.

08/1994-07/1997: Associate Dentist Gilbert M. Amthor, DDS Pasadena, CA

General dentist in very exclusive boutique type dental practice with a high emphasis on cosmetic dentistry. Patients were from the television and movie industry, and several patients were from the local Los Angeles professional sports organizations. Dr. Amthor was mostly absent when I was operating his practice. I gained great experience treating very particular patients. The quality of cosmetic dentistry that came out of that office was extraordinary.

Community Founder “Smiles for Christmas”. This was an annual program provided by my office with lots of

Projects: help from my staff, friends, and colleagues in the area. For the past five years, this one day totally

FREE clinic is opened on a Saturday in December. I opened my doors to whoever wished to have a

dental service provided to them. We offered a choice of a Prophylaxis, Amalgam or Composite Restoration, or Extraction. We collected no money, and no insurance is ever billed. It became a community gathering consisting of the local Police and Fire Departments as well as the local news.

This idea originally began to help only children in the community but for the last four years we have treated patients of all ages. The very first year we treated over 70 children and last year the turnout grew to just under 300 patients. We stayed as long as there were people to see and NO ONE was ever turned away.

Awards: Received numerous awards from several insurance companies as a result of superior patient service and consistent annual audit scores of 95% or higher. Delta Dental, United Concordia, Golden West, Safeguard, and PacifiCare, to name only a few.

Awarded a seat on a new committee at Golden West Insurance to evaluate and re-write the provider manual for their insurance plans.

Named one of “America’s Top Dentists” in 2007 by the Consumers’ Research Council of America.

Featured in “Inland Empire” magazine as part of their “35 Dentists to Love” article in March 2007. This article featured my office in Redlands, CA and I was given a full page in the magazine.
