Work Experience: LPN Transitions Lifecare Hospice- April 2025-Present, Patient Care Coordinator: In this role I am responsible for providing clerical assistance to the Hospice Team Leader and clinical staff. Duties include data entry into the patient’s electronic medical record, printing and tracking orders, obtaining prescriptions from physicians. Process routine admission orders; omit medications as directed by RN and fax to the pharmacy. Complete changes in patient data Netsmart. Remove unnecessary orders. Provide coverage for absent CTAs as needed. Assist with clerical office duties as necessary. Ensure documentation of patient care is complete, timely, and accurate in accordance with TL processes, policies, procedures and regulatory requirements.
