Urgent I-9 Document Submission Required

Dear Employee:

It has come to our attention that we have insufficient I9 documents on your

personnel file and the documents must be submitted urgently. Please be sure to sign and date your form, failing to do so will result in your form being invalid. Your document should be signed by your employer, which is Human Resources department of NYCSSS not your school location's Custodian Engineer.

Failure to comply with this request within two weeks may result in disciplinary action up to and including termination. We are missing the following documents below:

• Form I-9 Documents (SECTION 1 & 2)

• U.S. Passport or U.S. Passport Card

Or

• Copy Social Security Card

• Copy Identification Card

• ID card issued by federal, state or local government agencies or entities, provided it contains a photograph or information such as name, date of birth, gender, height, eye color and address

• School ID card with a photograph

• Voter registration card

• U.S. military card or draft record

• Non-US Citizenship

• Permanent Resident Card

• Employment Authorization Document Card
