I, agree to begin an assignment with Company in a position that includes PITO operation at a UPS facility beginning . I understand that although I am working at a UPS facility, I am still an employee of Company. Your employer and UPS expect that you work in a safe manner and operate equipment in a fashion that prevents accidents and injuries.

I agree to participate in PITO and site safety training with a UPS representative. This training will provide equipment and site-specific knowledge to the UPS assigned facility. I understand that my employer and I may be held liable for any accidents in which my behavior is determined to be negligible. I agree to these terms throughout my assignment at UPS.
