OSC Parent/Guardian Consent to Participate
I, the Parent/Guardian of, who is under 18 years of age, hereby grant permission for them to participate in One Summer Chicago 2025. I certify that I am the applicant’s parent or legal guardian and give them permission to participate in the program. I understand that the information provided by my child on their application will be shared with other entities for purposes of administering programs, research, and providing referrals to possible employers. I authorize the release of my child's application information to such entities and employers. I understand that I have the right to inspect this information and to request appropriate corrections through the administering agency. OSC Photograph, Social Media and Video Release Form I, the parent/guardian, hereby grant permission to the rights of ’s image, likeness, and sound of their voice as recorded on audio or video tape without payment or any other
consideration. I understand that their image may be edited, copied, exhibited, published, or distributed, and waive the right to inspect or approve the finished product wherein their likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of their image or recording. I also understand that this material may be used in diverse educational settings within an unrestricted geographic area.
Audio, photographic, social media or video recordings may be used for the following purposes:
● Conference presentations
● Educational presentations of courses
● Marketing for City of Chicago
● Informational presentations
● Educational videos
I will be consulted about the use of the photographs or video recording for any purpose other than those listed above. There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.
By signing this form, I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing this material for educational purposes.
