OBJECTIVE
Providing compassionate holistic care to the adult diabetic patient. I bring many years of experience to the patient as a registered nurse, advanced practice nurse and diabetes educator. As an advanced practice nurse I bring care that complements the medical team and enhances the patient experience. I have cared for and followed patients with, metabolic syndrome: diabetes, hypertension, hyperlipidemia, obesity as well as thyroid disease, adrenal problems and PCOS. SKILLS & ABILITIES
Utilized guideline directed medical therapy based on American Diabetes Association (ADA) and American Association of Clinical Endocrinology (AACE). The patient and I work together in a collaborative effort to make a plan to reach their goal A1C. We review the full array of medical options and adjusted medical plan as needed. I discuss the benefits, adverse effects and availability of potential treatments. I am well versed with the use of the newest diabetic medications & technology and inform the patients of the newest advances.

Besides seeing my own patients I had been seeing for Dr. John Tloczkowski’s I was tasked with calling these patients with test results and adjusting the plan as needed. I preformed follow up visits for Dr. Arora’s patients part of this past year as well. I was also the facilitation of diabetic supply orders for all four of the endocrinologist’s patients. II am also a certified diabetic educator and an active member of the Association of Diabetes care & education. Because of this training I had made it my job to impress on all these patients the benefits that they can have from maintaining diabetic diet and exercise program. I have made it my mission to inform all patients that they have the power to prevent complications of Diabetes. And as needed I referred patient with complications of Diabetes to the appropriate medical provider.

The plan of care was developed in conjunction with the patient based on exam findings, labs results, blood glucose logs, CGM reports and insulin pumps reports taking into account the patient experience and preferences it was then communicated at the office visit, tele-health visit, phone call and or My Chart.
