The specimens are submitted in two separate containers. A) The specimen is labeled with the patient's name and "aortic valve leaflets" and consists of a 3 cm aggregate of soft to hard, membranous to nodular, tan to pink valve leaflet segments, the largest 2.9 cm. Vegetations are suggested. Representative sections submitted following brief decalcification. B) The specimen is labeled with the patient's name and "mitral valve" and consists of a 3.2 cm in length soft and friable white valve leaflet with chordae tendineae attached. Focal nodularities involve the leaflet. Representative sections submitted following brief decalcification.

Comment:
The most significant inflammatory process involves the aortic valve leaflets in which fibronecrotic vegetations are populated by suspected bacterial colonies. A tissue gram stain and fungal stain (GMS) are performed on each specimen. The Gram stain for the aortic valve leaflets shows numerous microorganisms morphologically consistent with gram positive cocci populating the fibronecrotic tissues. The fungal stain is negative. The mitral valve leaflets reveal similar fibrotic nodules with focal areas of neutrophilia. The tissue gram stain is equivocal, and the fungal stain is negative.
