Responsibilities As an MDS coordinator I am responsible for the coordination of care for the residents in the SNF. This is achieved by assessing each resident on admission which includes but not limited to my physical assessment and inputting all data including CAAs, then quarterly until an annual assessment is required and the cycle begins again. This coordination of care includes not only my RN assessment but I must coordinator this care with each discipline such as the RD, rehab, social services restorative and any other type of care needed by this resident. The ARD is set and I must make sure staff is not only providing but also documenting the needs and care of this resident correctly. These assessments must also be inputted in matrix and sent to CMS in a timely manner. As MDS Coordinator I also am responsible for preparing the care plans needed for each resident and hold care plan meetings with staff from each discipline as well as residents and family members who wish to attend. The MDS position is a link between the direct care of the resident as well as financial reimbursement from Medicare, Medicaid, HMOs as I am responsible for all updates for skilled residents to Medicare but also medical updates to HMOs for review to make sure payment is received and skilled nursing care is necessary for the resident. To assure these payments I must acquire certification for skilled services from each doctor as well as provide all letters and information required to be sent to families / POAs when skilled services have ended. I also attend daily morning meeting with DON and unit managers to discuss current resident issues, weekly Risk meetings to monitor residents for any change in condition as well as Medicare meeting which is run by MDS. Previous to my position as MDS coordinator I was the charge nurse on a behavioral unit. I learned to care for individuals who not only had physical illness but mental illness as well. I was responsible for the care on the floor as well as the coordination of care with the CNAs. I worked with Doctors, and each discipline daily to achieve a high quality of care for the residents on my floor Accomplishments I have become certified as an RN. RAC -CT COORDINATOR.
