case management services are provided to the serviced population, especially in crisis

part of the Post Release and Home Study Program, it has been served and obtained
experience in offering Post-Release Services to the population we serve.
Maintain a case load in accordance with agency policies and procedures. Succeed on
independently maintain weekly contact with children and families and exercise
discretion and judgement, to continuously assess ongoing changes in behavior,
circumstances or conditions that may affect the minor' s safety. Assess and document
progress that children and their families make toward risk reduction, achievement of
services goals and positive outcome.
My role is to provide resources to the minors within the community and assist them when
needed to connect with schools, doctors and other institutions to facilitate their transition
in the country.
