Routine audits on Participants charts focusing on the following:
Informed Consent form and Process (ensure current version, count the number of pages.)
Study staff name, signature, date, time consent obtained, documentation process, confirm if copy given to participants)
Eligibility Criteria (match source documents with study protocol)
Scheduled tests and Procedures.
Missed visits, tests or procedures
Concomitant/Prohibited medications
Study Product administration/dosing
Clinical end point identification
Identification and reporting of serious Adverse Events (SAE) Expedited Adverse Event and Adverse Events (AE)
Compliance with unit and sponsor SOPs including Good Clinical Practice
Development of site Quality Assurance/Quality Control Tools
Identification of site staff training needs and conduct appropriate training e.g Good Clinical Practice, quality control and source documentation
