This investigation was conducted under the authority of TRIGOLD SECURITY AND INVESTIGATION AGENCY INC., as part of its standard protocol to evaluate and report on firearm-related incidents occurring within operational posts of assigned personnel

2.Matters Investigated

This report investigates the accidental discharge of a service firearm by Security Officer Juan Dela Cruz on June 10, 2025, at the MERALCO San Pablo Sector substation guard post. The matter under inquiry includes the sequence of events, possible procedural violations, root causes, and recommendations for preventive measures.
