Due to your leave coming to an end, if you are unable to return to work, your employment at 48Forty will end on Aug 21, 2023. Please note that this decision was not made lightly and that we have followed all appropriate legal procedures in coming to this decision. Your health benefits will end on Aug 31, 2023. You will receive information in the mail regarding continuation under COBRA in two to three weeks. All supplemental benefits, such as company paid life insurance, voluntary Life, dependent life, disability, flexible spending accounts and 401(k) contributions will end effective Aug 21, 2023.
