The role of the Care Coordinator in the transition of care setting is to assist patients with changing health care needs who also may require a variety of medical services. The Care Coordinator facilitates communication among medical professionals and ensures continuity of care connecting and establishing care with the primary care provider and ensure primary care specialists follow up as needed. Care Coordination is to improve communication with patients and medical staff regarding follow up plan of care, making sure the patient understands the next steps in care and meets with patient to assess and address barriers to care, increase positive outcomes and patient satisfaction.
