Cygnet Hospital Bierley is an hospital which has four different single-sex specialist wards providing high dependency complex care service for 18+ male and female individuals. These services include male low secure service for men, acute service for men, low secure service for women, acute service for women. Key Responsibilities include but not limited to:
• Supporting service users in monitoring their body temperatures, pulse rate and respiration, keeping track of diet and checking body weights, documenting in NEWS 2 score chart and escalating to the nurse in charge if needed.
• Ensuring that patients’ care is individualised (person centred care) and according to their PBS.

• Supporting patients to the community and ensuring their safety while they are off the hospital premises.
• Working alongside the Nurse in charge to provide high quality sustainable service, to make sure service users feel safe and well supported while in our service.
