• Provide care and companionship to 92 year old man who lived alone, fell often without help to get up for a few hours, needed assistance throughout the night as the family was unsure of his capability to be alone due to age.
• Assist the patient with ADL’s such as ensuring no falls while walking to and from restroom at night as well as throughout the day.
• Upon arrival massage ointment for bone pain relief onto patients hands for severe pain relief.
• Make the patients dinner or re-heat dinner if day nurse made any for patient that day.
• Administer daily vitamins and medication to patient before bed.
• Assist with nightly routine of patient such as brush teeth, use bathroom, change of day clothes into pajamas, and massage ointment once again onto patients hands for bone pain relief.
• Be in control of gate belt while patient transported self with walker around home.
• Clean patient, change and wash bed sheets if fecal or urinal accident occurred in the night.
• Check blood pressure and pulse once patient was on hospice.
• Walk behind patient with the use of a gate belt and walker to ensure of bodily movement and promote movement to decrease struggle of falls and movement.
• Assist in small workouts to enhance patients muscle strength.
• Feed patient by mouth once on hospice.
• Switch sides of patient while on hospice to prevent bed sores and uncomfortableness.
• Monitor the patient throughout the day and night.
• Record patients activities and medication time frames throughout the day and night.
• Give the patient company, provide genuine care and companionship to prolong lifespan as much as possible.
• Assist the patient in the morning with brushing teeth, washing face, and changing out of pajamas into day clothes.
• Grab the newspaper as the patient enjoyed reading his daily morning paper, make breakfast and administer daily medication.
• Hours were 4pm-8am Monday through Saturdays.
