RN Case Manager: for Aetna, Better Health of Kansas, a Medicaid MCO, managing technology, assisted waiver, physically disabled waiver, and Frail and elderly waiver with a caseload of 60 to 75 Medicaid members. Providing education and guidance to Medicaid members, navigating the waiver, benefits and management of chronic health care conditions, such as COPD, congestive heart, failure, diabetes, and mental health and wellness. In this capacity, I worked with independent, assisted, living, and nursing facility populations to provide healthcare and mental care support by assisting with obtaining DME, personal care assistance, and assisting members by collaborating with other healthcare professionals to ensure preventive healthcare maintenance was easily accessible. Hospice Registered Nurse Case Manager: for a Caseload of 8-15 management of End of life care and coordination of Intradisciplinary team to provide symptom management and needs of patient and family within the scope of Hospice services.
Health Care Coordinator: For a 96 bed Assisted Living Center. I oversaw the coordination of health care needs the of residents, management and training of 13 CMA, and 5 nurses. Coordination of admission and assessment of new admit clients in conjunction with Assisted Living Coordinator, Memory unit Coordinator and Marketing team. Assisted Marketing team with informational visits from potential clients.
Clinical Educator: Created annual curriculum for licensed staff and CNAs. New Hire Orientation Presenter, Clinical New Hire Educator and orientation scheduling. Maintained annual certifications for licensed and non-licensed staff, Staff immunizations, CPR recertification’s, Survey readiness, Infection Control Monitoring, QUAPI reporting, Directed In-service training, in use of correct nursing practice and documentation.
MDS Coordinator: Competent in MDS 3.0, Medical Unit Reporting. Managed the CMI for a 95 bed Long term mental health Facility, and 36 bed skilled nursing, 24 bed rehab Nursing building. Provided updates and coordination of care Managed Care patients with therapy and Nursing disciplines to maintain managed care requirements.
Weekend Supervisor: Manage Staff ensuring quality nursing practice provides supportive assistance, manage staffing needs, and manage Resident / family complaints. Relief Nursing: Provided Quality relief nursing as agency nurse for ICU, ER, Cardiac ICU, Telemetry Unit, and Behavioral Psych.
Electrophysiology Nurse: Provided pre/post procedure assessments, cardiac monitoring while providing moderate sedation during: cardiac ablations, Cardiac device implants, and cardioversions. Was integral in developing a INR to Heparin sliding scale for Atrial fibrillation Ablation.
