• Work with VCU Health System staff to identify eligible patients and appropriate time for patient screening

• Seek patients’ verbal consent to participate in the research study

• Assist patients as needed in completing the screening

• Work with ORW’s to provide assistance and education to patients in the community, clinical setting, and emergency department

• Help connect patients to community resource summaries for identified health-related social needs as appropriate
