Assess service needs of residents in HUD housing and link them to the appropriate providers and community resources. Facilitate programs and services such as health and wellness or transportation. Case Manager responsible for the coordination of community services for children and adults with chronic mental illness (CMI), intellectual disabilities (ID), brain injury (BI), and developmental delays (DD). Responsible for hiring and supervision of 85 staff and 70 respite care homes in a multi-county area. Consumer caseload of 165. Co-facilitate a weekly parent support and education group.
