July 2007 to March 2018: Bremer County Case Management

Case Manager responsible for the coordination of community services for children and adults with chronic mental illness (CMI), intellectual disabilities (ID), brain injury (BI), and developmental delays (DD). Responsible for the assessment of need and implementation of Individual Comprehensive Plans (ICP). Arrange for the provision of services, establish a therapeutic relationship with individuals served, and actively participate in their clinical care. Facilitate collaborative assessments, develop social histories and treatment plans, ensure that treatment plans are implemented, coordinate and monitor services. Continually evaluate the options and services required to meet the individuals’ health and human service needs.

January 2001 to July 2007: Lutheran Services in Iowa

Coordinator of Services for People with Disabilities responsible for the coordination of supported community living, adult rehabilitative option, and respite care services for adults and children with CMI, ID, DD, and BI in a twelve-county area. Develop individual treatment plans and monitor staff in implementing those plans. Track consumers’ progress on their individual goals and report their progress to case managers through oral and written quarterly reports. Responsible for hiring and supervision of 85 staff and 70 respite care homes in a multi-county area. Consumer caseload of 165. Co-facilitate a weekly parent support and education group.
