Develop, strengthen, and maintain partnerships and collaborations with internal and external partners.
Conduct active HIV/Stage 3 adult case reporting in assigned Michigan counties through a network of health care provider sites, hospitals, local health departments and clinical laboratories.
Review and process new and updated Adult Case Report forms received for data entry, check for duplicates, completeness and consistency of data as well as perform follow-up on missing and discrepant information.
Provide consultation to medical providers and local health departments, explain the reporting process and purpose, interpret reporting laws, and explain case definition. Identify and develop case-finding strategies at enrolled sites and assist with problem-solving. Determine sites requiring assistance with case reporting and additional training needs.
Scan and upload all paper case report forms into Content Manager, a digital record system utilized by the Michigan Department of Health and Human Services
Coordinate all reporting of HIV2 cases and other COPHI (Cases of public health importance) to CDC.
Coordinate with local and state health departments across the United States to provide accurate and complete data on the people living with HIV
Assign completed case to a disease intervention specialist for follow-up with the newly diagnosed client. This ensures the client is linked to a healthcare provider and prevents the spread of infection to others in the community. -Based Organizations, Hospitals and Outreach Activates.
