Nurse Case Manager
At admission, teaching patients and families about Hospice, comfort care, and the goal of Hospice.
Assess the physical and psychosocial status of patients and families within 24 hours of admission. (Alert, confused, lethargic or obtunded, breathing, pain, skin integrity…).
Explain about medication and comfort med.
Evaluate pain and adjust the pain med, SOB, and Oxygen.
Provide skilled nursing as determined by the interdisciplinary plan of care according to the Palliative Care Compass Guide.
Coordinate the patient's and family's care and services, including those of the team Social Worker, Chaplain, Volunteer, and Physician.
Reevaluation every week if the patient's condition is Hospice appropriate and report in the IDG (Interdisciplinary meeting). If not, the nurse case manager has to prepare to explain to the patient and family about the Live Discharge Extended Prognosis.
