Currently I have a disciplinary action on license that will go away once all criteria are met. I did not agree with this action but did not have the money to obtain proper help. I have taken the education units they wanted and just trying to finish paying fine. I have attached a letter of what happened and how I handled the situation. I still have good standing and my license has never been taken away or suspended in my 18 years of nursing. This is an embarrassment to myself and do not like that I have to tell anyone this when applying for jobs. I am a good nurse and my fellow staff and patients and their families have never had bad or negative things to say about my dedication and love of nursing. Please feel free to ask me any questions and again I have attached letter with my explanation. To whom it may concern,
For a period of time before going to the hospital to work, I was trying to still stay in home health at a different company as I could not stay at Amedisys after settlement for workers compensation came through for my injury of being attacked by a dog while on job at a patient's home. I started with Interim Healthcare and was glad because they were a good company and they used homecare home base to chart for patients like I was used to with Amedisys. Because we see patients in rural areas that there is not a lot around the internet with phone connections and our work tablets could go in and out depending on the weather, where you were for the visit and if the actual program was having problems on a certain day. There were times when I had to see patients with the system being down-charted on paper wound measurements and vitals and time in and out, then later when at home when I could use the tablet again I would put the information in at that time.
One of the days I was working one of my patients called and said they needed a visit and if someone could come. I had accidentally charted on the wrong person and turned my time in for the day and did not catch that it was the wrong patient due to similar areas of location and names. So it looked like I was falsely charted on people that were not seen and that is not the entire truth. I was let go and at the time I didn't know that I was turned in to the SC board on Nursing. Months later I was going through spam mail and saw an email form llr in SC but thought it was a scam because it was email and I never received a call or a letter in the mail. Couple months later I got another email, so I tried to call back the number on the email stating my name and that we could set up time to talk. I actually did this at least three times. I never received a call though or letter in mail, except months later when I had the court documents mailed to me for review to sign. In these documents it states that there was an investigation and that he talked to both parties involved and came up with his findings, when in doubt he actually never talked to me at all.
I did not have money for attorney and did not want to drag this out for years to come so negotiated what the board of nursing and lawyers for them that I would admit wrong doing and take online courses which I have completed and pay a fine which I'm still working on . When I get the fine paid it will be taken off record. Again I may have copied and pasted a nurse note sometimes, especially the first two sentences because sometimes it would be the same for the next patient, but turned in before I could add on to the note. I admitted to this and to the way I was documenting some visits when I got home so that this could end sooner.
I apologized for my mistakes and any complications that may have been caused. I did not manipulate and do anything on purpose.
