Roles and responsibilities while working as hospital pharmacist (India and Malaysia):

Compounding (when required), labeling and dispensing the medicines with required instructions to all kinds of patients.

Studying case sheets, checking adverse drug reactions, any medication or prescription error to dispense medicines to the inpatients by the indents raised by the Physicians or Surgeons, Resident and Nursing staffs.

Patient Counseling, dispensing medicines to the outpatients.

Dispensing medicines and guiding the patients the proper way of using medicines.

Analyze and identify customer needs perception. Instill brand name and confidence.

Monitor expiry dates, proper storage and supply to the wards.

Maintain stock, accounts and availability of medicines.

Guide and supervise trainee and Assistant Pharmacist.

Provide information about drugs as and when required by the doctors and Nursing staff.

Providing complete customer satisfaction with all necessary support.

Convince customers through knowledge, discipline and politeness.

Good Pharmacy practice and customer oriented.

Observe drug interactions and irrational prescriptions. Roles and responsibilities of the Pharmacovigilance job:

Preparation of project status and reconciliation reports.

Responsible for distribution of cases to the other pharmaceutical partners.

To review the cases that are processed by other Drug Safety Associate for accuracy in the data entered in the database as per source document, identifying clinically significant information missing from the initial report and ensuring its collection.

To attend team meetings, client teleconferences as required and to take sessions to the DSAs on recent updates in the process and to answer all the queries of case processors.

Responsible for coding all medical history events drugs/procedures/indications and laboratory tests according to the appropriate dictionary (For e.g., MedDRA, Company Product Dictionary and WHO-DD).

Responsible for writing medically relevant safety narratives of cases and checking the completeness and accuracy of data entered in the various field. Personal Profile:
