Reason for leaving: Temporary position. Needing something closer to home. State of North Carolina Department of Transportation – 1040 Prison Camp Road, Durham, NC 27705 – Supervisor Curtis Hornbuckle (deceased, unsure who is in the position now). Position: Transportation Worker 1 (reclassified from a 2) Responsibilities: Snow/Ice removal, dump truck operator, heavy equipment operator, storm removal, asphalt repair, filled in on dead animal removal truck, installed new driveways, pipe installation, material hauling dealing with all aspects to maintain streets and highways. Rate of pay: $18.00 per hour

Dates of employment: 2016 – June 2021.

Reason for leaving: Unhealthy work environment.
