• As needed: scheduled provider visits on behalf of clients, accompany clienct to visits, ensure access to other community and government-based services.

• Guided and teached care givers about symptom response plans

• Acted as an advocate and liaison between the patient/caregiver(s) and community service agencies (i.e. schools, Homeless shelters, hospitals, support groups, etc.)

• Provided ongoing follow-up, basic motivational interviewing, and goal-setting with clients/caregivers

• Along with other members of the team, conducted patient/caregiver screenings that include the medical, behavioral, and social needs to identify gaps in care and barriers to accessing resources critical to clients’ wellbeing.
