Interview families and children to assess family dynamic, mental health, behavioral, educational, and housing needs to determine the appropriate permanency goal and service plan.

Preparing and coaching youth to age out of the foster care system independently.

Providing support and guidance to children and families when they are trial discharged and completing the appropriate paperwork for final discharge from foster care.

Locate and provide a variety of resources to families to strengthen their family dynamics and family relationship.
