MAIN RESPONSIBILITIES AND DUTIES

● Admission assessment including pain assessment, fall prevention, diagnosis goals and intervention, care-plan, and discharge plan.

● Daily medication administration including Oral, IV, G tube and NG feedings.

● Blood sugar check and Insulin administration.

● Fall evaluation, fall incident reports, post fall neuro-check, adding relevant intervention r/t fall and implement as needed.

● Wound assessment, prevention and interventions, and implement. Wound treatment as ordered. Communicate with MD about new skin concerns.

● Carry out nutrition plan including dietary supplements.

● Supervise LVN, CNA and students while providing basic care and evaluating their needs.

● Communicate and educate family regarding medication and discharge instructions.

● Assess patients while participating in physical, occupational and speech therapy communicate with Rehab about their progression and notify MD.

● Time to time Electronic charting about patients their change of condition, fall incidents, new skin concerns, pharmacy and dietary recommendations, Medicare charting, and new orders etc.
