I worked in a primary care clinic and children with special needs clinic, where my primary role was phone triage using Dr. B. Schmidt’s pediatric triage protocols. I also rotated out to work in primary and special needs clinic. I did a great deal of case management with the special needs children and their families. I have experience in working with central lines, PICCs, ports and PIV. Experience with IVIG, ABX, factor 8 & 9 meds and TPN infusions.

I left this position to move to California to be closer to family.

October 2013 to January 2015: Community Health Clinic Nipomo, CA. Manager for adult and pediatric ambulatory clinics. Managing 10 MAs and one LVN in two busy clinics. Triage of walk ins and patient phone calls.

January 2015- January 2016: Sansum Pediatric Clinic:

Triage and clinic RN in a very busy pediatric clinic. left to move to Moorpark, CA for husband’s job.

January 2016 to August 2021: Home health infusion RN for Angel’s of Summit Home Health Care. Left to move to northern California.

September 13, 2021-to July 2022:

RN case manager for Home and Health Care management. Managing clients by phone and video calls, who qualify for HCBA state waiver program. Assessed applicants for appropriateness to qualify for HCBA waiver. Performed intake which includes plan of treatment, Menu of Services, consents and acuity assessment. Communicated with state nurse on patients as issues arose. Developed plan of treatments and care plans for chronically ill, disabled adults and children. Collaborated with social workers on client problems and possible solutions. Management decided to eliminate work from home positions, so I left to find a work from home position.

July 2022 to February 28.2023, : RN Case management for Palliative care patients. Vynca Care. This was a 100% work from home position. I managed 45 patients. Duties included assisting patient care coordinator with scheduling visits with each discipline. Made phone or zoom visits to each patient every 2 weeks performing physical assessments and providing education on symptom management, disease processes, medications, disaster preparedness and advance directives. Ordered supplies and DME. Facilitated referrals. Completed POLST with patients if they consented. Reviewed and updated care plans after every visit. Led weekly inter disciplinary meetings with provider and MSW. EMR was Athena and used Sales Force for data.

Laid off due to lack of census.
