Functions as a licensed primary health care provider responsible for managing health concerns and coordinating health care for acutely and chronically ill, mostly geriatric patients, in accordance with State and Federal rules and regulations, and the Nursing Standards of Care in a long term SNF. The patient population served included long-term patients as well as patients whose individual situation required a short term rehabilitation admission Direct contact with patients occurred exclusively in the SNF located in S Pittsburg TN known as “The Bridge”.
I functioned independently to perform age-appropriate history and physical for acute and chronically ill patients. Responsibilities included those typically expected of a Nurse Practitioner is such a setting and entailed the ordering and interpreting of diagnostic and therapeutic tests relative to patient’s age and condition-specific needs; prescribing appropriate pharmacologic and non-pharmacologic treatment modalities; implementing interventions to support the patient to regain and/or maintain physiologic stability; and monitoring the effectiveness of interventions. As needed, responsibilities also included but were not necessarily limited to facilitating the patient’s transition within the health care system including admission to services, referrals to specialists as needed/desired and discharges to private residences.
