Reviewing documentation for appropriate translating patient information into an alphanumeric and numeric medical code for information and reimbursement. Ability to maneuver within Quadra Med Software and the CPRS system as well as VISTA systems. Process the Austin errors that are on the daily 419 report and the VISTA errors that come up. Responsible for performing coding quality audits, providing feedback to coders and educating coders. Uses and understand the Internal Classification of Disease ICD 10 CM and PCS, and other coding references to ensure accurate coding and Diagnosis Related Group (MS-DRG and APR-DRG) assignment. Reviews, researches and processing CCI and VERA Edits. Provides coverage for coding by coding Inpatient Charts and Outpatient Encounters as needed for the service. Validating coding based on various Fee Schedules, CMS and VHA Coding Guidelines. Review Inpatient Charts for coding accuracy and all other errors that will cause process of completing the chart. Handle all Death Review and Audits for correct coding. Assist in the overall quality, education and completeness and accuracy of medical record documentation. Assist with making Standard Operation's for services that are in question and there is not clarity in the coding process. Perform reviews on the health record documentation.
