My responsibilities as a Case Manger included visiting assigned patients daily.

Visits consisted of assessing patients, monitoring medications, wound treatments, and reordering medications. Admitting patients and extensively reviewing patient’s chart and symptoms with hospice physician to determine hospice eligibility. However, my primary duty is to ensure my patients are comfortable during the dying process and that families feel supported and well prepared to care for their loved ones at home. This is possible with extensive education regarding medication administration, wound care, recognizing signs and symptoms of discomfort, and assisting with ADLs.

My responsibilities as a Medical Surgical Nurse included caring for acute patients daily. Administered medications via IV/PO/SQ/IM. Administration of blood and blood products.

Established IV access. Monitored and recorded input and output in patients with renal failure, gastrointestinal, and cardiac conditions. Mentored and coached nursing students and new graduates. Round with physicians/receive and follow through with new orders.
