Call in refills of medications in a timely manner. Receive and confirm monthly cycle medications and documentation. Assist with medical appointments, including making appointments, providing transportation, and preparing Professional Medical Order Forms and accompanying documentation.

Ensure that each person is supported under the recommendations of the Individual Plan.

Ensure that the home environment is clean, neat and presentable at all times according to agency policies and regulatory agencies. Maintain inventory of supplies and groceries. Submit maintenance requests.

Maintain contact with families and keep them abreast of all happenings involving their family member. Act as liaison to all agencies involved in services for the people living in the home. Public Health Advisor (RETIRED)
