planning and facilitation of linkages with community and center resources. Provided outreach

services to the person served in their natural environment. Followed clients with home visits and

supervision of proper medication and accompanied them to doctors and social services.

2004 — 2005

DRUG COURT CASE MANAGER, BROWARD COUNTY COURTHOUSE

Provided daily updates of reports regarding next day court calendar. Faxed rehab orders to

various providers for client admission to drug court programs. Reviewed stats and attended court

hearings on arraignment day. Maintained files and progress for out of state drug programs.

UNIFIED FAMILY COURT CASE MANAGER, BROWARD COUNTY COURTHOUSE

Performed Pro-Se tasks for clients without an attorney. Answered and handled all inquiries from

the families involved in the divorce cases. Reported to the judge assigned to the case. Reviewed

all Pro-Se files daily as well as coordinating and arranging pamphlets of various resource centers

available for judges to refer clients to the appropriate agencies available for their cases.

Domestic Violence Case Manager

Perform social service and court related tasks involving domestic violence. Conduct initial intake

and assess lethality interviews with the victims for the purpose of developing a case

management plan that will ensure the safety and well being of the victims. Assist in community

and social services referral. Monitor and maintain cases in which parties have been ordered to

attend a court-mandated program. Process restraining orders and various other court

documents.

Performed psychosocial assessment and assisted in the development of a service plan to help

towards the achievement of goals and objective of the client in order to have them become self-

sufficient.
