EXPLANATION OF TEMPORARY LICENSURE SUSPENSION AND FIVE YEAR BOARD MONITORING

In January 2015, my medical license was suspended for six months due to chemical dependency. (See purple highlighting on page 6 taken from medical board website). Up until this point, I was prescribed anxiolytics by my psychiatrist. However, in 2014 my marriage took a turn for the worse, as I was subjected to emotional and physical abuse. The volatility and unpredictability of my domestic life made it easier for me to justify taking more anxiolytics. Ashamed to tell anyone, I self-prescribed the same anxiolytics during the same time my psychiatrist was also prescribing them for me. In July 2015, 180 days after my license was suspended and after a vote by the medical board, my license reinstated (See green highlighting on page 6 taken from medical board website). My medicare and medicaid billing privileges were also reinstated after this time Despite period.having . In an order active to be medical considered license for by reinstatement, the middle of I 2015, first had I remained to meet under all of the the board’s board’s supervision requirements. until October 2020. (See green highlighting on page 6). During this period, I had to meet all mandated stipulations, in order to maintain my good standing. These regulations included random drug screens (2-4 per month), attendance at 10-12 AA meetings per month, quarterly in person meetings with members of the medical board in Columbus, Ohio, quarterly meetings with a board approved physician, in addition to a peer monitor.

Throughout this five year span of supervision, I NEVER had a positive drug screen. However, in February 2020, I was cited for not submitting my required quarterly documentation, which I had misplaced. (See blue highlighting page 5-6). The penalty for this clerical oversight, was pending the board’s hearing.

Due to the pandemic, the boards decision was delayed for three months. In May 2020, the members agreed to a monetary fine for this non-drug related offense. There were no further conditions placed on my license nor was my monitoring extended. (Please refer to orange highlighting on page 5 and 6 of board documentation). After meeting all requirements set forth by the medical board and after a hearing by the medical boars, was I was released from all supervision in October 2020. (See rose colored highlighting page 5). Today, I am in a much better place both mentally and emotionally. I am finalizing my divorce and I take steps every day to ensure that my physical and mental health are optimized so I can be the best father and physician that I am capable of. 4 of 7
