Job Responsibilities
Coordinate project management activities, resources, equipment and information.
Break projects into doable actions and set timeframes.
Liaise with clients to identify and define requirements, scope and objectives.
Assign tasks to internal teams and assist with schedule management.
Make sure that clients’ needs are met as projects evolve.

Job Responsibilities
Coordinate and facilitate patient care through assessment, case planning, service provision and implementation.
Communicate patient needs to a variety of care team members through referrals, linkages and follow up accordingly.
Manage discharge plans upon completion or graduation from the project.
Work collaboratively with all OVC subpopulations, Caregivers, physicians, and nurses to ensure high quality care
Act as the patient's advocate as it relates to services coverage and other assistance

Key Responsibilities
The primary role was to assess, advocate and planning of care for individuals and provide patient education and support them through health and illness. However, there are various other responsibilities that I undertook in the Rapid Assessment Unit as well as in the Wards, including to:
Responsibilities:
Provide critical care nursing in the unit; responsible for continuous monitoring and assessment of assigned patients
Maintain and record accurate and detailed flowsheet of patients condition

Consult and coordinate with healthcare team members to assess, plan, implement and evaluate patient care plans in a dynamic critical care environment
Perform procedures of intravenous therapy including venipuncture

Worked with Diginitas International as a District OVC Programs Coordinator in Zomba and Phalombe with the core duties of Planning and implementation tasks, training and mentorship tasks, coordination tasks, monitoring and evaluation tasks, reporting and conducting studies for evidence based research. Worked with Diginitas International as a HBC coordinator for Zomba, with the core duties of providing technical support to the district health offices and Dignitas district team with planning, implementing, monitoring, evaluating and research studies in high impact conditions under the integrated TB/HIV/AIDS prevention, Treatment, care and support in line with the national TB/HIV management in Malawi and mentorship to all health facilities in the district to MOH and other partners in good practice in the management of TB/HIV AIDS prevention, Treatment, care and support.
