DISSERTATION FOR FELLOWSHIP FCPS II (CARDIOLOGY)
Residual ischemia after streptokinase therapy: This study included 346 patients admitted with acute myocardial infarction, out of these 137 received streptokinase. After discharge patients were followed up for a period of 6 months and a treadmill stress test performed after 6 weeks of myocardial infarction. The exercise characteristics of the patient who received and did not receive streptokinase were compared.
PRACTICAL EXPERIENCE (starting from recent followed by previous ) MEDICAL & SURGICAL INTENSIVE CARE UNIT at King Abdul Aziz University Hospital Jeddah: Following are the primary responsibilities at ICU
• Duties at ICU in King Abdul Aziz University hospital is rotation between Medical & Surgical ICU at 3 monthly interval & at time monthly intervals. Since Feb 2006, start of Cardiac surgery at King Abdul Aziz University hospital, taking care of post op cardiac surgery is also part of our responsibilities during our rotation in Surgical ICU
• Elective & emergency endotracheal intubation
• Mechanical ventilation.
• Management of most of the medical emergencies including shock, ARDS, sepsis, respiratory failure, neuromuscular emergencies, autoimmune, infectious and cardiac emergencies.
• Putting in lines i.e central venous, arterial line, pulmonary artery catheter, Quinton catheter for dialysis & CRRT and temporary transvenous pacemakers.
• Management of patients post op after general surgery & specialized surgeries e.g Cardiac surgery, neurosurgery, plastic surgery, orthopedic surgery & gynaecological surgeries
• The registrar on duty in ICU is team leaders for CPR team all over the hospital except in Pediatric department.
• Patient admitted to hospital, who deteriorate clinically, are also seen & if required, are transfered to ICU for further management.
• Assessment and management of critical patient in emergency department.
• In addition emergency Hemodialysis & continuous renal replacement therapy for septic & non septic patients.

• Indoor patient management,
• Handling cardiac emergencies presented in Emergency of Mayo hospital Lahore on 24 hours basis when on call.
• Particularly arrhythmias, myocardial infarctions (post MI mechanical complications), tamponade etc

• Non-Invasive testing were a part of the training, including Holter monitoring, tread mill stress testing, echocardiography both transthoracic and transesophageal, SPECT scanning. Also dobutamine echo stress testing (both by the transthoracic and transesophageal approach)
• Assisting and at time independently carrying out Invasive investigations: coronary angiography, PTCA, Balloon mitral, pulmonary and Aortic valvotomy, temporary and permanent pacemaker implantation
• Cardiac Surgery: Postoperative management after Cardiac surgery i.e. Coronary artery bypass surgeries, valvular surgeries and surgeries for congenital heart diseases. MEDICAL EXPERIENCE at Mayo Hospital Lahore, Pakistan
• Taking care of Indoor patients.
• Bedside Medical procedures like pleural aspiration, liver biopsy, renal biopsy, pleural biopsy.
• Carrying out abdominal and pleural ultrasound.
• Assisting in gastroscopies and bronchoscopies.
• A Rheumatoid clinic was also managed during the training in Medicine & a tetanus ward managing 6 patients.
• 36 hours Emergency per week in one of the most busy hospital of Lahore Pakistan i.e. Mayo hospital Lahore, handling about 300-400 patient per day in emergency.
• Medical intensive care unit with experience in intubation and handling with complication of mechanical ventilations
