I trained at the Cleveland Clinic in Ohio, In ICU including surgical ICU and Cardiothoracic Post-Surgical ICU. My schooling is well known for their medical training and student’s excellence. Relevant Work Experience:
Current assignment is Internal Medicine at Desert Palms Medical Associates. Working with very sick and compromised adult and elderly population. Majority of my patients seen in this setting have multiple comorbidities, and cancer. I manage diabetes types I and II, insulin pumps, diet education for obese patients and diabetic patients. I do steroid joint injections, knee, hip, shoulder, wrist, ankle, heel, and greater trochanteric. Diagnose and treat musculoskeletal disorders, such as low back injuries order MRI verses x-ray for compression fractures, determine if they need vertebroplasty verses PT, shoulder injuries frozen shoulder, rotator injuries and degree ect. Botox for migraine headaches, occipital nerve blocks for occipital headaches, or PT for neck pain. I refer to a lot of patients that need surgical repair (as last resort unless severity suggest sooner) to neurosurgeons. I improved office compliance numbers with Humana Wellness and Medicare Wellness within the AOC to go from 11th in the area to 3rd in the area in less than 6 months as I was the sole provider on this project.
December 2018-April 2020 Beacon of Hope Hospice. I assess and determine patients that are appropriate for hospice are. I do face to face assessments to help determine if patients are appropriate to stay on hospice. I order pain medications and treatments to help hospice patients stay comfortable during the transition period. I do all the recertifications to stay on hospice. I help with wound care orders, and on call duties in my rotation.
January 2018- March 8, 2019- Arizona Institute of Medicine & Surgery (AIMS) Family Practice. Family practice medicine, Botox injections for chronic migraines, steroid joint injections, manage HTN, DMTII, I have a lot of elderly patients with multiple chronic conditions, COPD, Cancer patients, remove skin lesions, lancing abscess, suturing, and running urgent care in my rotation. Seeing patients of all ages, immunizations, well child exams, sports physicals, acute illness exams of all ages including ear infections, streptococcus throat, lice, common colds and influenzas, virus illnesses that did not deem antibiotic treatments. I did toenail extractions for ingrown toenails, skin lesion removals for shave biopsies, rashes including herpes, shingles, ring worm, fungal rash in elderly, hernias, broken bones like feet, toes, fingers, arms, ankles, legs, dealing with CHF verses pneumonia exacerbations, diabetic crises in attempt to keep patients out of the ER, and much more.
September 7, 2015-Decemeber 23, 2018-Kingman Regional Medical Center (KRMC) Pain Management, and Musculoskeletal Clinic. Manage patients with acute or chronic pain. Examining and ordering appropriate images on areas in pain such as: neck, back, knees, shoulders, ankles, and hips. Interpreting and going over the images including MRI’s with patients. I also order appropriate injections, referrals to neurosurgery or orthopedic surgery, and conservative interventions. Injections I order for the collaborating physicians I work with include: ESI, Facet, transformational, bursal injections, occipital nerve blocks, and trigger point injections. I do all the Botox in the clinic for chronic migraines, about 3-4 per day. I can do occipital nerve blocks, trigger point injections, including the PSIS region; I can do bursal injections, and knee injections. I see 80-100 patients in 4 ten- hour work days. I am fast pace and able to stay on task. I am in the office most of the time on my own, as the physician’s I work with are part time.
July 15, 2014- July 2015 Stow-Kent Family Practice, Stow, Ohio. Work as a certified nurse practitioner (CNP) collaborating with 2 Physicians, while evaluating/assessing, diagnosing, and treating patients of all ages independently with both acute and chronic illnesses. I am excellent at obtaining patients history and physicals, I am good at diagnosing hypertension, diabetes type 2, hypothyroidism, cardiac murmurs, and cardiac problems such as heart failure, I am good at treating these disorders as well as the daily sore throats, skin rashes, bug bites, ear infections, and common illness that arise acutely like bronchitis. I am excellent at medicare Wellnesses, and well child visits, and physicals for sports.
June 28, 2007 – July 1, 2014 Cleveland Clinic, Cleve, OH. Worked on the CCICU with critically ill cardiac patients. Stabilize, optimize, and maintain hemodynamically unstable cardiac patients. I took care and assisted with placement of Swan Ganz Catheters, EKG, IABP, A-line, CVVHD, Alsius cooling of cardiac arrest patients, LVAD, and ECMO certified with experience as well as diabetic care certified. I have taken courses on pressure ulcer and skin care, as well as courses on intense diabetic care.
January 2004 – January 2005 Arbors of Canton LTACH Canton, OH. Worked on LTACH unit as an LPN taking care of patients in multi-system failure, and rehab able patients. Passing medications to a high number of patients through peg tubes and orally, administration, care of peg and J-tubes, managed and took care intense pressure ulcers, and surgical wound care, colostomy bag care, and care for elderly and middle-aged patients.
April 1997 - June 2006 Laurel Lake, Hudson, OH. Worked first as an STNA then became an LPN in 1999. As an LPN I passed medications to 33-38 residents, was charge nurse on day shift of the unit, delegated duties to STNA’s, took vitals and charted on the skilled residents, charted on other residents on a event specific basis, helped feed the patients that where feeds, administered aerosol treatments through the nebulizer machines.
Certification Wound certification class in October 2017. I am one of the 1300 Nurse Practitioners in America to be wound certified. Certification includes diagnosing, treating and managing wounds that Include chronic and acute wounds, diabetic ulcers, vascular ulcers, pressure ulcers, and other various wounds. I can debride wounds both by autolytic and mechanical means.

Experience included obtaining pertinent CC, HPI, medical history, examining both focused and head to toe, diagnosing, and treating conditions that are seen in primary care offices, however, all these patients at this office had severe mental illnesses that included but not limited to schizophrenia, bipolar disorders, personality disorders, anti-social disorders, and obsessive-compulsive disorders, therefore care had to be in collaboration with each patient’s psychiatrist.
